Functional Outcomes Referral Policy
Thank you for your interest in obtaining a booking with Functional Outcomes.
Our booking policy is described below:
If the clinician has availability, the referral source will be booked into a spot on the assessment scheduling calendar.
Clinicians at Functional Outcomes strive to take a balance of plaintiff and defense referrals. Due to a high demand for defense assessments, these requests are automatically wait listed. As dates are made available in the clinicians’ schedules, law firms who have placed an individual on the wait list are contacted and offered a date. Once the retainer fee is received, the booking is confirmed. While we attempt to meet requested timelines for assessments, unfortunately there is no guarantee of obtaining a booking when placed on the wait list.
Other occupational therapists can be located on the British Columbia Society of Occupational Therapists website (www.bscot.org) if we cannot meet your time frame requirements for an assessment.
When phoning to obtain a booking at Functional Outcomes, please have the following information ready for our administrator:
- Client (Patient) Name
- Date of Birth
- Type of Assessment: Functional Capacity Evaluation, Cost of Future Care Evaluation, or Critique
- Law Firm (lawyer, legal assistant, and contact number)
- Trial Date and Date Report Required
- Date of Injury
- Type of Injury/Diagnosis (e.g. soft tissue, chronic pain, brain injury, orthopaedic, psychological, etc.)
- Pre-Accident Occupation
- Home Location (for Cost of Future Care Assessment)
