Life Care Planning Evaluation

To our clients:

A Life Care Plan is designed to identify your current and future disability-related needs, such as therapy, equipment, and home support services. This evaluation will either be conducted in your home and/or at the Functional Outcomes clinic; your counsel will notify you of the location of the evaluation. In either location, this will involve an interview and functional testing. The purpose of functional testing is to more clearly and accurately document your functional abilities and specific limitations related to your injury.

Important points to remember...

  • Loose, comfortable clothing (bring running shoes; bring shorts if knee/ankle injury)
  • Please do not change your medication regime prior to your testing
  • No caffeine or cigarettes one hour prior
  • Please bring or have available any adaptive aids you are currently using (i.e. knee braces, wrist splints, etc.)
  • If you are driving for assessment at the clinic, there are two adjacent parking lots allowing full day parking

Location:

Our clinic is centrally located at 1755 West Broadway. Clinic facilities are fully wheelchair accessible, with elevator access to the second floor from the main entrance. Full day parking is available in the lane behind our building (between Broadway and West 8th Avenue) at a number of different lots. One available full day parking lot is Menkis Tower Parkade, at a cost of $10.00 per day.

Transit access is available via the #99 Broadway B-Line (Express) Bus which stops nearby at Granville street, or via the #9, which stops directly in front of the clinic. Both these busses are accessible from the Commercial Street Sky Train station.
